League MVP Shai Gilgeous-Alexander led OKC with 30 points.
The defending champs shot 56% from the floor and hit 23 of 47 three-pointers.
Cleveland shot just 37 percent and committed 21 turnovers.
Center Jarrett Allen was -31 for the game, while Donovan Mitchell and Jaylon Tyson were each -26.
Darius Garland (toe) missed the game for Cleveland (24-20), which now heads to Charlotte for a Wednesday game with the 16-27 Hornets.
