On Sunday night, it was a third successive year City lifted the prestigious title, a feat no other club has ever achieved.
Jafonso and Matias Bonanno overcame Nottingham Forest and Tottenham Hotspur in the quarter-final and semi-final to tee up the showpiece final against Leeds United which saw former team-mates Bonanno and Tekkz go head-to-head.
First, Joao provided Mati with a 3-2 lead on aggregate, but the second leg between the holding ePrem duo was a lot more chaotic.
Three times Tekkz took the lead and three times Mati responded in 90 minutes and extra-time.
With the score 6-6, it was difficult to see who would find the final strike.
Up stepped our experienced Argentine who scored two late extra-time goals to seal an 8-6 triumph and a historic third title.
