Levi Colwill scored the only goal of the game to ensure Chelsea will be making a return to Champions League football next season.
While they missed out on the Champions League, Forest will make a return to Europe themselves in 2025-26, as they will play in the Europa Conference League.
The Blues will return to the top table of European football having finished the season strongly under Enzo Maresca.
