Unai Emery's side looked well set to record a second consecutive victory over MLS opposition having seen off St. Louis 2-1 last week.
A goal either side of half-time from Ollie Watkins and Donyell Malen had Villa ahead. Head coach Emery made a raft of changes, including eight at the interval, and Nashville delighted most of the crowd of 20,000 with two goals in the final 15 minutes to level.
Villa replaced all of their starting XI, with a run out for all outfield players. Midfielder Jacob Ramsey back involved from the off after missing the contest with St. Louis. He was part of a strong starting line-up, with additions as half-time substitutes including Ezri Konsa, Tyrone Mings, John McGinn, Youri Tielemans, Morgan Rogers and goalscorer Malen.
A lively start saw both sides go close and Watkins had already seen several openings by the time he headed in from Lucas Digne's fine cross on 18 minutes.
Ramsey was denied on half hour and a pass from Emi Buendia.
Villa youngster Jamaldeen Jimoh-Aloba fed Malen to find the far corner after the hour amid several changes.
Matthew Corcoran halved the deficit with 15 minutes left and Englishman Sam Surridge equalised for the delighted locals.
Villa face Roma at Walsall's Bescot on Wednesday before friendlies at Marseille and Villarreal next Saturday and Sunday respectively.
