Robin Roefs has returned to full fitness and replaces Melker Ellborg from the team that kicked off the derby win over Newcastle United prior to the international break.
Nordi Mukiele and Reinildo Mandava are also back in the Black Cats' starting side, filling the full-back positions on either side of Luke O'Nien and Omar Alderete.
The pair replace Lutsharel Geertruida and Trau Hume, who both drop down to the substitutes' bench this afternoon.
The final Black Cats change sees Enzo Le Fee return to the starting XI, having set up the derby winner over Newcastle after leaving the bench at St James' Park.
Le Fee will start on the left-hand side against Spurs, with Chemsdine Talbi back among the substitutes.
