The Senegal international is set to become the latest player to leave the Hammers after their relegation from the Premier League.
Last season, the 21-year-old was one of the standout stars for the Irons, but he looks set to join the Bees after just one year at London Stadium.
Brentford have reached an agreement with West Ham for Diouf's arrival.
The left-back featured in the opening two games of the campaign under Nuno Espirito Santo, but has been left out of the squad since.
There has been a sense since the East London club's relegation that the Senegalese ace would be one of the players to leave and his exit has felt inevitable.

Brentford's interest in Diouf has been well-known and they're about to win the race for his signature.
The Hammers paid an initial £19million to Slavia Prague last summer for the left-back.
According to Sky Sports journalist Lyall Thomas, Brentford will now pay £35m plus £5m in add-ons to sign the Senegal international.
The Irons have clearly made a decent profit on Diouf, who also attracted interest from one other club.

There are still some final details and a medical left to conclude, but the defender is keen on a move to the Gtech Community Stadium.
